Responsibilities:
The Patient Care Advocate works with patients to identify their own individualized needs, resource connections and assist with follow up treatment.
The Patient Care Advocate maintains open communication between all parties involved in patients' treatment to ensure the highest quality of care is obtained with close collaboration and minimal interruptions in care.
Assist Center and Regional Management teams with patient retention services for multiple clinics.
Maintain all medical reception procedures and communicate to team members updated procedures when necessary.
Procedures include but are not limited to patient retention, patient intake/discharge, phone systems, documentation, Prescription Monitoring Program, patient scheduling, Electronic Medical Record encounters, Releases of Information, and related communication with treatment partners, etc.
Collaborates with patients, providers, office support staff, ancillary departments within the company, and community agencies to maintain accurate and timely data exchanges between all entities involved in the patients' care.
Coordinate's patient appointments, referrals, and transportation needs for both CleanSlate and community appointments as needed.
Obtains appropriate releases for each referral that is completed and conducts follow-up for referrals made.
Scheduling new and recurring patient appointments for mid-level provider (PA or NP), physicians and other clinical providers.
Maintains effective, pro-active, and superior follow-up communication with patients, providers, office staff, and ancillary departments within the company and community.
Attends regular meetings or conference calls, including but not limited to involving Interdisciplinary Teams and collaborating community agencies, completing all required documentation in the EMR.
Scanning of patient documents into Electronic Health Record All other tasks or projects as assigned.
Qualifications:
This position is subject to a background check for any convictions directly related to its duties and responsibilities.

This position must successfully pass a pre-employment drug screening Bachelor's Degree in a Human Services field preferred.
Excellent written and verbal communication, organizational, and customer service skills.
Knowledge of community resources, networking, and referral processes.
Experience in a Case Management, or Care Coordination setting.
Knowledge of and proven experience navigating and documenting in an EMR system.
Ability to work independently with minimal supervision.
Ability to handle multiple tasks simultaneously and prioritize based on severity of need Must have reliable transportation What we offer and perks! The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
